What is on the form
- Header block: company, client site, date, cleaner name, supervisor name
- Each area from our checklist (lobby, offices, restrooms, break room, floors, trash, closing) with its check items
- Score boxes 1 to 5 per area, with the rule printed on the form: 1 means redo tonight, 2 below standard, 3 meets the contract, 4 above, 5 is the training standard
- Fix list section: any score under 3 gets written here with a due date
- Issue line for the complaint log: date, problem, reclean date, fix sign-off
- Cleaner and supervisor signature lines
What the form is not
This form is a record of one inspection visit. It does not certify cleaning quality, satisfy any contract clause by itself, or replace a supervisor's judgment. No cleaning mandate requires it; it exists so you have an answer when a client asks what was found and what was done.
Photo rules if you attach pictures
No photos of people. No photos in restrooms, locker rooms, exam rooms, or any space where people expect privacy. No photos of personal items, documents, screens, mail, or medication. Photos show completed work areas only, taken when the area is empty, and only with the client's permission. Read the full photo and privacy rules.
